Registering for iHealthSpace establishes a secure connection with our practice so that you can privately provide us with the information we need to ensure you or your loved one receives the best possible care.

This one-time registration will set you up to electronically receive any future patient questionnaires and pre-visit forms from the practice. Registering also allows us to save the information you provide and fill in applicable information on future forms, to save you time and prevent you from having to repeatedly answer the same questions.

1. Select 'For Myself' or 'For Someone Else' to begin your iHealthSpace registration.
2. Follow the steps to enter patient and caregiver information and to create a username and password.
3. Accept the Terms of Use and Privacy Policy.
4. Check your email for a message from ihealthspace@partners.org with the subject 'Thank you for Registering with iHealthSpace.'
5. Click the link in the email to activate your account. Now you are ready to log in and complete forms from the Down Syndrome Program!

Our pre-visit questionnaires help us gather and organize the important information we need to provide you or your loved one with the best care possible. Our team reviews and discusses the information you provide in advance of your visit so that we are best prepared to meet your needs.

We know that these forms might seem lengthy, but our medical team reads all of the information you provide so that your time spent with us can be maximally efficient and productive.

Once you have registered with the MGH Down Syndrome Program in iHealthSpace, you can visit www.ihealthspace.org to log in to your account.

At the top of your home page, you will see notifications for any pending forms. You can also access a list of your forms by selecting 'Forms' from the blue menu bar at the top of the screen.
